Where to go.. who to see?
Answer: In order to do this you will need to file the Durable Mental Health Care Power of Attorney form. You can find this form on the Arizona Attorney Generals Office Website, it is in a PDF format. The form does require a notary and a witness. The website where you can find the form is http://www.azag.gov
